THE Ministry of Health (MOH) has set the end of 2027 as the deadline to complete the implementation of Electronic Medical Records across all primary healthcare facilities, as part of an ambitious nationwide digital transformation of the public health system.
Health Minister Datuk Seri Dr Dzulkefly Ahmad said the initiative is being driven through the deployment of a Cloud-Based Clinic Management System for health clinics and a Dental Information System for dental clinics.
“The installation of CCMS began in 2024 and by the end of 2025 will cover 412 health clinics. For 2026, the ministry is committed to digitising a further 137 health clinics, and the remaining 588 will be fully digitised by the end of 2027.
“Similarly, for the installation of DIS, 120 dental clinics had been completed by the end of 2025 and the remaining 458 will be fully digitised by the end of 2027,” he said in a parliamentary reply to Mohd Nazri Abu Hassan (PN–Merbok), who had asked about the ministry’s comprehensive plan to digitalise daily operations, data integration and system interoperability between public and private healthcare facilities.
Beyond primary care, the ministry is also expanding digital systems across hospitals through the Total Hospital Information System, which has been in place since 1999.
To date, 23 hospitals have completed installation.
“For this year, the ministry is committed to digitising 16 hospitals, while the remaining 113 hospitals will be fully digitised by the end of 2029,” Dzulkefly said.
In parallel, the ministry is intensifying efforts to consolidate national health data through the MySejahtera application. The platform has so far integrated 30 million vaccination records, seven million prescription records, three million dental records and 2.8 million health screening records.
In the near term, a further 90,000 medical imaging records will be incorporated, advancing the ministry’s “One Individual, One Record” aspiration.
“This initiative is part of the ministry’s digitalisation agenda to strengthen continuity of care, enhance the efficiency of healthcare service delivery, reduce duplication of treatment and ensure faster and more secure access to information.
“At the same time, to create a holistic healthcare ecosystem encompassing both the public and private sectors, private healthcare facility operators are being engaged through the standardisation of interoperability standards.
“The involvement of private healthcare facilities will continue to be strengthened through the standardisation of work processes and interoperability standards to elevate the level of digitalisation in the national healthcare sector to a higher level,” he said. - February 27, 2026
