KUALA LUMPUR – Rapid Rail Sdn Bhd has one week to submit a preliminary report on the incident involving a door malfunction on an LRT train on Monday, said the Transport Ministry.
Minister Datuk Seri Wee Ka Siong said in a statement that the Land Public Transport Agency (Apad) had given the train operator seven days or before August 2 to submit an initial report, followed by a full investigative report with supporting documents within 21 days or by August 16.
“Further action on the licensed operator of the rail service involved in the incident will be decided based on the examination of the complete report related to the incident.”
Wee said Apad will open an investigation paper on the incident under the Land Public Transport Act 2010 (Act 715).
He said under Act 715, safety is a priority condition stipulated under the licence of the rail operator, and that all land public transport operators and any personnel involved in providing rail and train services must make safety a paramount aspect of the service.
The 3.01pm incident on Monday saw doors to the TR44 train remain open after departing the Damansara LRT station heading towards the Lembah Subang station.
Wee said the train was carrying seven passengers at the time but no injuries were reported.
He added early findings indicate that the incident had resulted from a “Door Close Confirmation Unit (DCCU)” that failed to operate normally.
“However, it is too early to ascertain the actual cause of the DCCU’s failure as investigations may lead to other conclusions related to maintenance or adherence to the train operators’ standard operating procedures when addressing technical issues.
“Rapid Rail was also instructed to undertake short- and long-term measures to ensure such an incident does not recur in any of Rapid Rail’s train operations.”
Wee said Apad had immediately taken action by directing Rapid Rail to perform a detailed investigation into the incident and submit Train 44’s maintenance reports to Apad from before the incident occurred.
“It is regrettable that this incident has occurred and the ministry and Apad will not tolerate any element of neglect. It must be stressed that any person that fails to comply with safety requirements is liable to legal action.” – The Vibes, July 28, 2021
